2. What personal data do we process?

We process your personal data because you come to us or are referred to us with a healthcare request, and/or because you provide them to us yourself. We process, among other things, the following data:

  • First and last name, gender, and date of birth.
  • Address details, phone number, and e-mail address.
  • Special and/or sensitive personal data, such as your Citizen Service Number (BSN) and the medical data necessary for your treatment.
  • Insurance information such as your health insurer, insurance number, and policy or coverage.

3. Purposes and legal basis of data processing

We process your personal data exclusively based on the following legal grounds:

  • The performance of our treatment agreement.
  • Your explicit consent.
  • Compliance with a legal obligation (such as maintaining medical records).

We process your personal data for the following purposes:

  • To carry out your medical treatment and to maintain your patient file.
  • To be able to call or e-mail you if this is necessary to carry out our services.
  • To handle your payment or the claim with your health insurer.
  • To comply with legal obligations, such as data we need for our tax returns.

4. Which systems do we use in our practice?

To offer you safe care and to optimally protect your data, we use strictly secured software systems:

  • Electronic Patient Record (EPD) and client portal: We use Zorg GGZ (provided by Axians) for the secure recording and management of your medical file. Our client portal is also part of Zorg GGZ.
  • Questionnaires and eHealth: For diagnostics and to measure whether our treatment is effective, we use questionnaires via QuestPro (provided by Axians). We use these results to continuously improve the quality of our care. If we share this data with national quality institutes, we do so exclusively pseudonymized or with your explicit consent.
  • Prescribing medication: For the electronic and secure prescription of your possible medication, we use the Electronic Prescribing System FarMedZ (provided by FarmedVisie).
  • Digital referrals: For securely receiving, sending, and processing referrals (for example, from your general practitioner), we use the secure healthcare platform ZorgDomein.
  • Secure communication: For the secure digital exchange of medical data and confidential messages with other healthcare providers, we use ZorgMail (provided by Enovation).
  • Regular e-mail traffic: For e-mails from and to , we use Proton Mail, a service with strong security and encryption. Please note: message traffic between us and you may not be fully secured if your own e-mail provider does not offer comparable security. We therefore advise you not to send medical content via regular e-mail.

5. Sharing personal data with third parties and Professional Secrecy

As a psychiatrist, your practitioner Cristina Zwaan is bound by medical professional secrecy (confidentiality). This means that everything you discuss is treated strictly confidentially and that data is never simply shared with others.

Cristina Zwaan Psychiatrie en Psychotherapie only shares your personal data with third parties if this is necessary for the execution of the agreement and to comply with any legal obligation. We sign a processing agreement with the companies that process your data on our behalf (such as Axians) to ensure the same level of security and confidentiality.

  • Healthcare providers (such as general practitioner and pharmacy): For good and safe treatment, we exchange data with your referrer (usually the general practitioner) at the start and completion of the treatment, after your explicit consent. If we prescribe medication for you, we share relevant medical data such as possible allergies or intolerances with your pharmacy to monitor medication safety.
  • Family and relatives: Sometimes it is useful for your treatment to involve your family or relatives. However, we only exchange data with them when you have given your explicit prior consent.
  • Health insurers and billing: To get your treatment reimbursed, we submit claim data to your health insurer (for contracted care) or send the invoice to you (for non-contracted care). This claim or invoice includes, among other things, the category your complaint falls under (the care demand classification / zorgvraagtypering). If you object to this you can sign a so-called 'Privacyverklaring Zorgprestatiemodel' (Privacy Statement Care Performance Model). We will then no longer mention the care demand classification on the invoice or claim.
  • Administrative and financial service providers: For both the claim with health insurers for contracted care as well as the billing to you for non-contracted care, we use the billing service of Axians.

6. How long we keep personal data

We do not keep your personal data longer than is strictly necessary to achieve the purposes for which your data is collected. We apply the following statutory retention periods:

  • Medical file: we are legally obliged to keep your medical data for at least 20 years after the end of the treatment.
  • Financial administration: invoices and payment details are kept for 7 years for the Tax Authorities.

7. Automated decision-making and Cookies

Cristina Zwaan Psychiatrie en Psychotherapie does not use automated decision-making. Our website also does not use cookies or similar technologies.

8. Transfer of your file

Are you switching to another practitioner? Then it is important that your new practitioner is aware of your medical history. We will then safely transfer your file, but we exclusively do this after you have explicitly requested us to do so.

9. Your rights: view, modify or delete data

You have the right to view, correct, or delete your personal data. In addition, you have the right to withdraw your possible consent for the data processing or to object to the processing of your personal data by Cristina Zwaan Psychiatrie en Psychotherapie. You also have the right to data portability, which means that you can submit a request to us to send your personal data in a computer file to you or another organization.

You can send your request to . To ensure that the request for inspection was made by you, we ask you to send a copy of your identity document with the request. In this copy, make your passport photo, the strip with numbers at the bottom of the passport (MRZ), your passport number, and your Citizen Service Number (BSN) black to protect your privacy. We will respond to your request as quickly as possible, but no later than within four weeks.
